We are Doctors of Physical Therapy, we are athletes and artists, we are teachers and trainers, we are purveyors of healthy movement choices, and most importantly, we are on your team.

By focusing our skills on manual medicine, movement and motivation we are uniquely qualified to provide you with the highest level of clinical, service and care excellence. As such, we crafted this practice specifically not to be in-network with any insurance company. This allows us to serve you with one-on-one care for 60 minutes without any restrictions ... well, sort of. (See the Direct Access page for more on this.)

We accept payment in full at the time of service in the forms of cash or check. Sorry, we do not accept credit cards. Checks are to be made out to "Movement Thru Rehab". We will email you a receipt for services rendered.


Physical Therapy Initial Evaluation :: $180

  • 1.0 hour of one-on-one care
  • Thorough history-taking, movement assessment, physical exam
  • Discussion of movement impairments, goal- and expectation-setting
  • Application of objective outcome measures to track progress
  • Initiation of treatment plan involving a mix of manual therapy and/or movement retraining
  • Development of an initial and tailored Home Exercise Program (HEP) for functional goals
  • Corollary correspondence to advance clarity, compliance and communication

Physical Therapy or Wellness Follow-up Sessions :: $160

  • 1.0 hour of one-on-one care
  • Re-assessment of movement impairments and treatment plan appropriateness
  • Continuation of manual therapy + movement paradigm as warranted
  • Review of questions and correction of HEP technique
  • Progression of HEP as appropriate to patient’s status and ability
  • Tracking progress through objective outcome measures

Wellness Evaluation Level 1 :: $160

  • 1.0 hour of one-on-one care
  • Functional Movement Screen® (FMS) – testing of fundamental movement patterns
  • Tailored report of FMS results and discussion of interpretation
  • Initial strategies for addressing improvement of movement patterns
  • Y-Balance Test® (YBT) – testing of lower body or upper body strength, flexibility, and control
  • Discussion and interpretation of YBT results
  • This is only for athletes/movers who desire performance enhancement NOT Physical Therapy
    (i.e., if you are not currently experiencing pain)

How to recuperate some of your costs

If you are receiving patient services and have out-of-network Physical Therapy benefits we will provide you with a receipt known as a “Superbill”, which is the typical requirement for insurance members to submit to their insurance company for reimbursement. Reimbursement can be partial or full, depending on your insurance plan. Please know that you may be liable for a different or separate annual deductible and a different coverage agreement than with in-network Physical Therapy providers. Once you have met this deductible your insurance company will begin coverage under out-of-network terms. 

Call your insurer and ask the following questions:

  • What are my out-of-network (OON) Physical Therapy benefits?
  • Do I have a separate OON deductible to meet?
  • What is my out-of-pocket maximum?
  • Is there a per-visit limit or dollar limit?
  • Do I need a referral from a physician?

Once you have this information, provide us your insurance card information to include on the Superbill and submit it with the claim form found on your insurer's website.
